Full time
Location - Inner West (Travel required within areas such as Balmain, Concord and Strathfield)Flexible hours and days – Monday to Friday, Full time or part time; no shift work!
Pay rate – $54.47 per hour + superannuation
7 additional leave days per year
Pay less tax with Not For Profit Salary Packaging
Travel will be reimbursed at 0.88 cents per km in line with our travel policy

We're thrilled to announce that our Aged Care Assessment Services ('ACAS') team is expanding, creating exciting opportunities! ACAS positions offer flexibility and autonomy and are based across both metropolitan and regional locations, with most roles involving local travel within each region. We have an exciting opportunity for a Clinical Aged Care Assessor to join the team.
What will your role look like?You will be conducting assessments to identify and meet the complex needs of clients and refer to appropriate aged care services under the Aged Care ACT.
Day to day this includes:
Collaborating with clients, families and multidisciplinary teams as requiredWorking collaboratively with colleagues to promote a supportive and positive team cultureConducting accurate aged care assessments using the Integrated Assessment Tool (IAT) to determine client needs and eligibility for home support servicesReviewing support plans to meet client's changing support needsWorking effectively both autonomously and as a member of a broader team in delivering to key performance indicators within identified timeframesAbout Catholic Healthcare Limited (CHL)Catholic Healthcare Limited (CHL) is an Australian not-for-profit organization that provides a range of healthcare and aged care services in accordance with Catholic values and principles.
