We believe in empowering individuals to live their best lives, and we need a skilled manager to help us achieve this vision.Key Responsibilities:Client Planning and AssessmentEnsure client planning and assessment aligns with service standards and legislative requirements.Develop and implement personalized care plans that cater to individual needs and preferences.Conduct timely reviews of client plans and assessments to adapt to changing needs.Embed clinical governance frameworks into everyday practices.Service Delivery ManagementOversee the timely and high-quality delivery of services in compliance with funding guidelines.Monitor team performance and productivity to meet service delivery targets.Implement efficient scheduling processes to optimize workforce capacity.Resolve service delivery issues promptly to maintain client satisfaction.Review programs annually or as guidelines change to ensure best practices.Quality Assurance and ComplianceEnsure adherence to regulatory and organizational quality standards.Lead quality audits and internal reviews for service improvement.Ensure compliance with Aged Care and NDIS Quality Standards.Drive innovation and continuous improvement in service planning and delivery.Staff Leadership and ManagementLead and develop the service delivery team through guidance and training.Foster a positive work culture with clear communication of roles and expectations.Collaborate with HR to ensure effective staffing and onboarding processes.Conduct performance appraisals and provide coaching to enhance staff development.Client Relations and Stakeholder EngagementBuild strong relationships with clients, families, and external service providers.Monitor contractor compliance and service quality delivery.Act as the primary contact for escalated client concerns, ensuring effective resolution.Represent STAR Community Services at client meetings and external forums.Develop innovative strategies to enhance client satisfaction and experiences.Reporting and Data ManagementMaintain accurate client records, service delivery reports, and care plans.Monitor and report on key performance indicators (KPIs) for service delivery.Prepare regular reports for the COO on service outcomes and staff performance.Participate in developing reporting tools as necessary.To be considered for the role, you must respond to the Key Selection Criteria:Proven experience in managing client care, planning, and service delivery in aged care, disability, or community services.Track record of delivering high-quality, timely services in the Home Care sector with strong client satisfaction.Demonstrated ability to manage budgets effectively.Excellent leadership skills with a focus on mentoring and team development.Strong problem-solving and conflict-resolution abilities.Experience in client relationship management and service improvement initiatives.In-depth understanding of Aged Care Quality and NDIS Standards, clinical governance, and relevant regulations.Awareness of the current reform agenda across the Aged Care and Disability sectors.What will you bring to the role:Expertise in Client Needs Assessment: Leverages a background in Social Science or Allied Health (or a related field with 3 yrs commensurate experience in a clinical model) to assess and address the complex needs of vulnerable clients, ensuring tailored service delivery that aligns with individual goals and requirements.Strong Communication and Relationship Building: Utilizes well-developed interpersonal skills to effectively engage with a diverse range of stakeholders, fostering collaborative relationships and navigating challenging interactions with empathy and professionalism.Autonomous and Team-Oriented Leadership: Demonstrates the ability to work both independently and collaboratively within a team, driving high-quality service provision while adhering to established procedures and guidelines.Effective Organizational and Time Management Skills: Employs strong organizational skills to manage multiple priorities efficiently, ensuring timely service delivery and compliance with record-keeping requirements and reporting standards.Commitment to Continuous Improvement: Embraces a mindset of continuous improvement, actively seeking opportunities to enhance service quality and adapt to changes within funding models and organizational goals, while remaining aligned with the mission and values of STAR Community Services.What can STAR Community Services Offer you?Full Time Role based in ClevelandCompetitive salary package + benefit of NFP salary sacrifice options.
