You need to be a good listener and communicator and have an empathetic attitude.Key components of the role include:Building and maintaining a great network of referrers who will help you achieve your targetsMeeting clients and their families and understanding the supports they needCreating person-centred support plans that reflect the client's needs and aspirationsEnsuring that clients and referrers have a positive experienceUndertaking client risk assessments to keep everyone safeHeaps of 'feel-good' times. We get to really make a difference, and that feels amazing!Our head office is located in Mulgrave, in the heart of the south-east suburbs, and we also have an office in Geelong– so when you're not visiting clients, you will work between the office and home.Mandatory Requirements:Valid and current Victorian Driver's LicenceNDIS Worker Screening Clearance and Working with Children Check (or willing to obtain)NDIS Worker Orientation ModuleCurrent First Aid and CPREmployment with CareChoice is subject to satisfactory background checks which include Professional References, Employee Working with Children Check, and NDIS Worker Screening Clearance.
