About the Role:We are seeking an experienced CHSP Coordinator to join our team, working directly with clients to facilitate service onboarding, assist with ongoing service coordination, and manage program requirements.
This role is ideal for someone with a background in CHSP who is passionate about supporting clients in various service areas, including transport, social support individual (SSI), social support group (SSG), home maintenance, domestic assistance, and flexible respite.
Key Responsibilities: Work with clients to onboard them into the CHSP, ensuring smooth access to services.Assist clients with navigating and coordinating their specific service needs.Undertake all necessary paperwork, database updates, and compliance reporting as required.Maintain and nurture strong, positive client relationships to support ongoing satisfaction and engagement.Collaborate closely within a team-oriented environment, providing support to achieve the program's outcomes.About You:To succeed in this role, you will need to have previous experience working within the Commonwealth Home Support Program and an understanding of its requirements.
You'll be comfortable with flexible scheduling, whether part-time, full-time, or spread over the week, and excel in team collaboration.
