This recruitment may be used to fill future positions via an eligibility list for permanent, full time, part time, temporary and casual positions. The Clinical Midwife Consultant Grade 2, Complex Care Coordinator will provide a complex and diverse midwifery consultancy service to patients and health care teams across CCLHD that enables the provision of contemporary, high quality, person-centred clinical midwifery practice related to women identified with level 3 psychosocial vulnerabilities and for pregnancies requiring complex care planning.
As a Clinical Midwifery Expert and leader, you will: Be the central point of contact for other clinicians and services working with women with complex care needs.Facilitate client engagement with existing local services to enable smooth discharge/transition to other services.Implement and maintain the principles of "Keep Them Safe" as described by the NSW Department of Premier and Cabinet.Facilitate the development and coordinate the management of clinical pathways, according to current evidence based best practice.Participate in the planning, implementation, and evaluation of care to patients based on evidence and recognised standards.Ensure appropriate strategies are utilised to facilitate services to persons with cultural and linguistic diversity and to Aboriginal and Torres Strait Islanders.Facilitate, coordinate, and chair multidisciplinary complex case planning meetings for identified vulnerable pregnant women and their families.Work in partnership with services to ensure seamless transition of care for pregnant women with level 3 psychosocial vulnerabilities.Training and supervision of midwives who are utilising the psychosocial screening tools.Develop and evaluate plans for the role and a model of care for women with complex care needs in the perinatal period.About you Current registration with the Australian Health Practitioner Regulation Agency (AHPRA) as a Registered Midwife with at least 5 years full time equivalent post registration experience.Demonstrated skills in working with women experiencing severe and complex psychosocial vulnerabilities.Demonstrated knowledge of support services available to women in the perinatal period.Demonstrated ability to provide leadership that facilitates the ongoing development of evidence based clinical practice.Where you'll be working The CCLHD provides the community of the Central Coast with a caring, comprehensive, and patient centred environment dedicated to improving the patient journey.
