Service work involves partnership with individuals and families to identify goals, develop strategies and evaluate outcomes.This may involve a range of activities/functions including: Assessment, Advocacy, Interpersonal Support Work, Co-ordination, Case Management, Community Development and other such responses that facilitate the achievement of specified goals.We are seeking case workers who will work as part of our KEIHS Team: Keys to Early Intervention Homelessness Service, an early intervention case management program working with individuals and families to support at-risk tenancies within the communities where KEIHS is co-located with host agencies.Experience and knowledge in the housing and homelessness sector is essential for this role.
Knowledge of client management systems such as QHIP and/or other databases are highly desirable, as well as case management experience.Salary and conditions are in accordance with the Social, Community, Home Care and Disability Services Industry Award 2010, Level 4 ($41.52 - $44.68 per hour).
