The Division's main objectives are: To provide safe, professional, person-centred care and support services tailored to individual needs with strong focus on our customers. To perform excellent customer service and care duties in line with funding and mandatory industry requirements safely in customers' homes. POSITION DESCRIPTION The Out of Hospital Care team provide high quality Case Management and Coordination services to people who are frail aged with complex needs, people living with a disability registered with the NDIS, patients discharging from a NSW public hospital under the ComPacks, SASH and Palliative Care programs and various other funded programs providing Case Management and/or Coordination to vulnerable members of our community.
The primary objective of this position is to undertake assessments and ongoing management of customers referred to LiveBetter for a variety of programs delivered by LiveBetter Out of Hospital Care Team. The role will comply with all requirements of the relevant program guidelines and be responsible for the arrangement and ongoing monitoring of brokered services as required.
RELATIONSHIPS Reporting to the Out of Hospital Care Program Manager. Working closely with all Coordinators, managers, team leaders and staff. Building and maintaining relationships with all external stakeholders. MANDATORY QUALIFICATIONS AND EXPERIENCE Current unrestricted Australian Drivers Licence and willingness to undertake travel in accordance with responsibilities of the role. Current First Aid and CPR Certificates. Completion of NDIS Worker Orientation Module 'Quality, Safety and You'. Diploma Community Services – Case Management or relevant qualifications (Nursing / Allied Health etc) and a willingness to attain Case Management Qualifications if you do not possess Experience in comprehensive assessments and/or Case Management. Experience in the use of customer management software e.g. Total Client Manager (TCM). Desirable Understanding of the issues facing the frail aged and disadvantaged in the community. SKILLS, KNOWLEDGE, AND ABILITIES Strong negotiation, advocacy and networking skills. Sound level of skill utilising computerised technology including Microsoft products. High level written and verbal communication ability including influencing, listening, negotiation and conflict resolution. Strong time management skills with the ability to complete multiple tasks with competing priorities. DUTIES AND RESPONSIBILITIES Provision of accurate customer reporting. Provision of case management to eligible customers within the geographic areas serviced by LiveBetter Case Management Services. Promotion and development of Case Management Services. Providing education to other service providers about LiveBetter Case Management Services. Conducting skilled and comprehensive assessment of customers and developing a suitable individual plan with the customer. After hours and on call responsibilities may be required as part of the OOHC Contract Requirements. Maintain Current & UpToDate Vaccinations as requested by NSW Health in line with Program Requirements. RISK Staff are required to be aware of the LiveBetter's risk and opportunity management principles and contribute towards building a strong risk management culture. This includes:
Undertaking operational responsibility to perform tasks and duties diligently and effectively, contributing to effective operational risk and opportunity management. Bringing to the attention of their managers/supervisors, risks, and opportunities within their areas of operation which may not be well mitigated and may affect the performance and reputation of LiveBetter. HEALTH, SAFETY AND ENVIRONMENT RESPONSIBILITIES All staff are required to take reasonable care for their own health and safety and that of other personnel who may be affected by their conduct. Advise LiveBetter in writing of any conditions which may impact ability to carry out the responsibilities required of the role.
NATIONAL POLICE CHECK, NDIS SCREENING CHECK, AND WORKING WITH CHILDREN CHECK The position holder is required to pass and hold a current National Police Check, a NDIS Screening Check, and a Working with Children's Check (NSW/VIC) or Blue Card (QLD).
PRE-EMPLOYMENT MEDICAL ASSESSMENT The position holder is required to pass a pre-employment medical assessment, including drug and alcohol screening.
