About the Opportunity
Melbourne City Mission (MCM) is a leading community services organisation that innovatively works alongside thousands of Victorians and their communities to overcome barriers experienced through homelessness, early years, disability, palliative care, early childhood intervention services and education.
Frontyard Youth Services is a specialist youth service providing a range of multidisciplinary programs to meet the holistic needs of young people aged between 12 and 25 years who are at risk or experiencing homelessness. Frontyard aims to support young people to meet their physical, emotional and social needs and to develop pathways out of homelessness.
This full-time position is located onsite and supports young people who are residing in the Refuge and will enhance our Healing Informed Case Management response to young people with Circuit Breaker's collaborative case management team. The position will work in a multi-disciplinary team moving from an individual to team case management model of care. The position will also provide some outreach and transition support to young people.
About you
We are looking for someone who can:

Facilitate and support a co-working team case management approach with the multi-disciplinary team including accommodation support workers, senior mental health recovery worker, and senior practice leader to young people through a healing-oriented framework responses to young people who are homeless or at risk of homelessness.
Provide onsite and outreach support to young people that enables them to achieve their goals.
Develop, maintain and review living skills plans that help young people create positive and sustainable pathways out of homelessness.
Work with team members to facilitate access to specialist support services, including health, mental health, AOD and therapeutic interventions both within Frontyard and through external services in accordance with the case plan.
Maintain an up-to-date knowledge of youth support programs including services that provide homelessness responses to young people.

Essential:

A bachelor qualification in youth work/social work or a related tertiary qualification.
Demonstrated experience working within a case management framework and accommodation/refuge setting.
Understanding of assertive outreach support and interventions, including settlement support with a sustaining tenancies practice lens.
Understanding or experience working with young people at risk or experiencing homelessness, including knowledge of healing-oriented approaches.
Strong passion for working with young people and ability to engage young people actively and assertively with complex needs.
An extensive understanding of the homelessness service system with knowledge of patterns, trends, systemic issues and principles in working with at risk young people.
Knowledge of the legislative requirements when working with at risk young people.
An understanding of the requirements for ensuring child safety.
Excellent communication and problem-solving skills encompassing interpersonal, verbal and written, and negotiation skills.
Provide First Aid certificate.
Strong organizational, time management and computer skills.
Availability to work on-site and in the community Monday to Friday standard working hours.

Screening Criteria:

National Police check.
Working with Children check and NDIS Worker Screening check or ability to obtain.
Valid Victorian Drivers licence.
Right to Work in Australia.
