Full Time role supporting clients across Dubbo & Parkes Region Commencing base salary $73,539 + 11.5% Super + tax free salary packaging up to $18,550/year. Company pool car available or be reimbursed at $0.96 / km when own car used. Here's what your day could look like as a Case Manager:
Forge meaningful connections with your clients, shaping their journey towards holistic wellness. Craft personalised assessments and goals, measured by outcomes that celebrate strengths. Manage budgets and care plans aligned with client choices, ensuring their autonomy and comfort. Embrace a dynamic week—traversing your designated region, collaborating with health professionals, and balancing office days from home. Drive initiatives to expand our reach and advocate for our exceptional services. To thrive in this role, you'll bring:
Qualifications in Case Management, Aged Care, or related fields. Experience in case management, paired with a knack for tech tools like Microsoft Word, Excel, and Outlook. A driver's license, along with the confidence to work autonomously and as part of a team. What's in it for you?
