The Pre and Post Release Support Service (PPRSS) in Alice Springs offers a unique chance to make a tangible difference in the lives of adult prisoners preparing for release and reintegration. As a Case Manager, you will provide tailored support to individuals three months prior to their release and throughout their transition back into the community. This role focuses on addressing critical challenges such as housing, addiction, family connections, and employment, helping participants build essential life skills and reduce the likelihood of reoffending.
Your work will involve developing culturally appropriate Reintegration Care Plans, providing intensive case management, and collaborating with stakeholders to deliver meaningful outcomes. * We strongly encourage people who identify as an Aboriginal or Torres Strait Islander to apply * Your Key Responsibilities Provide intensive, person-centered, and culturally sensitive case management to offenders with complex needs, ensuring individualised Reintegration Care Plans are created and regularly reviewed.Respond to referrals, complete assessments, and manage intake processes, ensuring accurate documentation and compliance with organisational protocols.Advocate for participants and coordinate referrals to appropriate services to address housing, addiction, employment, and other identified needs.Conduct risk assessments, facilitate therapeutic interventions, and lead case conferences with key stakeholders to ensure optimal outcomes.Deliver outreach services and support participants transitioning to independence, promoting self-management and access to required supports.Build and maintain strong relationships with participants, internal teams, and external stakeholders, contributing to the program's success.Perform administrative tasks, including maintaining accurate client records, preparing reports, and ensuring data integrity in compliance with organizational standards.Requirements for Success With a significant background in correctional services/justice system and/or Community Services sectors, you will be confident in your case management and culturally aware assessment processes. Other requirements include:
Relevant degree or diploma in community services or equivalent, with at least one year of experience working with individuals facing complex challenges.Demonstrated expertise in person-centered, strength-based case management and a sound understanding of prison release and reintegration processes.Proven ability to provide therapeutic interventions and manage a caseload of clients with diverse and confronting needs.Exceptional negotiation and advocacy skills, with a strong commitment to empowering participants to achieve their goals.Experience working collaboratively within multidisciplinary teams and building effective relationships across diverse inter-organizational networks.Strong cultural competency, particularly in working with Aboriginal and Torres Strait Islander communities, and sound knowledge of their cultural strengths and needs.Proficiency in administrative tasks, including client data management systems, and the ability to produce high-quality written and verbal communication.Full driver licence with no restrictions.Before starting work with us, you will need to undertake a national police history check, qualifications, and referee checks, and have a clearance to work with children/vulnerable people.
