Make a lasting difference The Role Centacare is offering a part-time, fixed-term Case Manager position until June 30, 2025, as part of our Outer North Youth Homelessness Service program.
This role operates independently within the Barossa, Gawler, and Playford regions, with regular support and guidance from a team based in Elizabeth.
Team meetings and ongoing collegial support will ensure you're connected and supported in your role.
You will work closely with young people aged 15-25 who are homeless or at risk of homelessness, helping them build essential independent living skills, explore housing options, access educational resources, manage health needs, and connect with local community services.
Additionally, you'll collaborate with community-based services in the Barossa Council area to assist young people in finding transitional housing and other vital resources.
Key responsibilities: Complete intake assessments to understand each young person's needs and goals.
Provide crisis support and conduct regular outreach to engage at-risk young people.
Work with clients to set goals and track their progress through case plans.
Oversee tenancy for six supported properties, ensuring compliance with tenancy regulations.
Liaise with community and government services to secure housing and support for clients.
