The role is based at Wayville but works largely in the community undertaking active outreach. Proven experience supporting Aboriginal and/or CALD clients is essential, candidates with Aboriginal and/or CALD heritage will be prioritised.
Both Full and Part Time positions available.
You will have significant active outreach experience, be a confident and capable case manager, able to undertake assessments, develop and manage against support plans, have excellent case noting (ISBAR), and make best use of your networks to proactively access services and supports.

Above Award leave entitlements, which are 22 days annual and 12 days sick leave per annum (pro rata), and salary sacrifice is applicable.
