As a Case Manager, you will work directly with caregivers, the elderly and families under the direction of the Coordinator. This role involves delivering social work and case management services to achieve the goals of Better Together's programs.
Your work at this level requires a sound knowledge of program activity, operational policy, the Human Service Quality Framework and other relevant legislation, and service aspects of the work performed within your team.
Position DescriptionFacilitate community development initiatives and collective solutions.Create and maintain client case management plans by conducting initial screenings, intakes, and holistic assessments in consultation with clients to determine, document, and coordinate access to client-led services and supports.Conduct assessments and provide interventions in the community, including home visits, counselling, and service coordination, with a focus on addressing the client's psychosocial needs.Evaluate the effectiveness of case plans, making adjustments as needed and proactively addressing current and future needs.Facilitate referrals to appropriate services and supports based on client needs, ensuring a seamless transition by coordinating follow-ups and maintaining communication with service providers to monitor progress and outcome.Collaborate effectively with cross-functional teams within the Better Together network, fostering open communication and leveraging diverse expertise to deliver the best possible outcomes for clients. This includes aligning goals, sharing insights, and ensuring seamless integration of services to address client needs holistically and drive exceptional results.Set priorities and monitor workflow in your own area of responsibility including performance indicators from funded programs and Better Together.Operate computer database systems to provide advice, reports, and assistance, ensuring timely and accurate documentation of all case management activities.Report on the progress and outcomes of your responsibilities, providing backup support when necessary to maintain service continuity.Coordinate with service providers to arrange and monitor client services, ensuring their ongoing engagement and support.Organize and participate in case conferences, meetings, and team discussions to ensure effective collaboration and client care.Advocate for Better Together programs and services in the community, building strong relationships with service providers and stakeholders.Maintain high standards for client data management, ensuring client contacts and records are accurate and up to date.Contribute to the development and evaluation of programs, ensuring alignment with quality standards and continuous improvement.Deliver culturally safe services, ensuring that all clients feel respected and supported in their needs.Collaborate with external service providers to monitor the appropriateness and effectiveness of support services provided to clients.Qualifications RequiredRelevant Tertiary Qualification in Community Services, Human Services, Social Work or related discipline.Driver's Licence.Capacity to successfully acquire a Blue Card and undertake a Police Check clearance.Travel and overnight stays at delivery regions will be required from time to time.First Aid Certificate.Knowledge and SkillsKnowledge and experience of contemporary principles and practices of social work and case management.Demonstrate local community connections with the care finder target population or a specific sub-group within the target population.Ability to attain a detailed understanding of the range of aged care supports and services that are available in the local community, the process to access and the requirements needed.Summary of role requirements:Flexible hours available.1 year of relevant work experience required for this role.Working rights required for this role.Expected start date for role: 15 February 2025.Expected salary: $97,377 per year. #J-18808-Ljbffr
