Case Manager - ACT (Internal Opportunity)Due to significant growth, we are looking for a Case Manager! This is an exciting opportunity to work with an independent, not-for-profit organisation located in Mawson, working with a small, friendly, and client-focused team. As well as Case Management with our Home Care Package clients, you will also be actively involved in service promotion, providing advice on access to community-based aged care to community members, and working closely with our team of wonderful direct Support team members.
