At Aged Care Deloraine, we are committed to creating a fulfilling life for our people.
We are currently seeking applications for a Care Manager position overseeing our two Residential Aged Care facilities in Deloraine. Kanangra and Grenoch are close-knit facilities with 48 and 46 beds, respectively. We are a small rural community-based NFP organisation.
The successful applicant will have full responsibility for the delivery of excellence in clinical care to our residents. You will have the ability to drive great clinical, social and emotional outcomes for our residents. Working as part of the management team, you will help drive improved outcomes for our residents.
As our Care Manager, you will be an influential leader with a drive to develop the capacity of our Clinical Leaders and entire Nursing and Care Teams. You will be walking into an organisation with solid leadership and processes, and your role will be to work with the executive to oversee quality, risk, and education.
At Aged Care Deloraine, we continue to invest in our Clinical Leadership team resources as we know the roles come with huge responsibility, and they drive better outcomes for our residents. We also know that in this role, we provide greater support and assistance with workflows for our nursing, L&L and care teams.
We currently have a Care Manager. However, we have identified the need to create a new Care Manager position, and our existing Care Manager will step into a new role and act as the chief clinical leader for the organisation. The position will report to this chief clinical leader, who drives the strategic direction of all operations, including clinical.
We have a Clinical Care Coordinator (CCC) at each facility. We have 24/7 Registered Nursing at both facilities and are proud that we operate above the RN Care Minute targets. We have dedicated additional software that focuses on individual risk management for each resident. This software enables us to identify gaps more effectively, but more importantly, it enables a structured corrective action plan and process that has accountability built in at every point.
We have a solid clinical governance structure and systems in place. Both of our facilities have recently been re-accredited with full accreditation (no unmets) and have the full 3-year accreditation period provided.
We also have a supportive and professional Board Clinical Governance Committee that meets regularly.

About you: Are you an experienced Care Manager or Registered Nurse with sound clinical knowledge and contemporary practice? Do you have a demonstrated ability in workplace engagement, empowering & motivating your teams, building a steadfast culture, and ensuring best practice in addition to clinical competence and resident-focused care? Do you have knowledge of sound clinical governance systems? Do you have an eye for detail? Can you work collaboratively within a team environment? Are you methodical? Do you like a challenge? Are you a people person who can build rapport with anyone? Are you passionate about aged care and want to play a role in leading aged care reform? Do you enjoy a challenge and thrive in a fast-paced environment? Experience in a Leadership or Management role in the aged or healthcare sector would be highly desirable. A sound understanding of the Aged Care Quality Standards and the ability to actively monitor the ongoing successful delivery. Experience managing Clinical Risk and Quality Outcomes of individuals under a Clinical Governance Framework. Be an effective communicator with excellent customer service skills and demonstrated problem-solving skills. Be highly efficient and organised with proven time management skills. Demonstrated experience in coaching and mentoring staff. Demonstrated experience in educating staff. Current Nurse Registration with the Australian Health Practitioner Regulation Agency (AHPRA). Evidence of right to work in Australia. Hold a valid driver's license. Fully vaccinated against COVID-19 (3 doses). A current National Police Certificate with no precluding offences that satisfies the Aged Care Act 1997 or willingness to obtain. A current Working with Vulnerable Persons Check with NDIS endorsement or willingness to obtain. Able to satisfy the suitability matters for Key Personnel working in Aged Care.
