We currently have an opening for a dedicated and detail-oriented individual to join their Community Team as a Care Manager for their new CHSP program.
Key Responsibilities:Managing the Commonwealth Home Support Program (CHSP) across the North, East, and West Melbourne regions, as well as Gippsland, VictoriaEnsuring the delivery of person-centred care support services to clients in line with Aged Care Quality StandardsEngage with a multidisciplinary team of support staff in the delivery of servicesEnsuring a smooth administration of new client intakeManaging the assessments and coordination of servicesPrerequisites required for the role:Tertiary qualifications in Aged Care, Social Work, Nursing, or a related field (minimum Diploma level; Bachelor's preferred).Experience within an aged care environmentStrong understanding of the Commonwealth Home Support Program (CHSP) and related legislation, as well as the broader aged care regulatory framework.Strong Management and Leadership experience essentialExperience with service navigation, referral management, MAC Portal and data entryExcellent time management, communication and negotiation skillsCurrent NDIS Working Screening Check and Victorian Drivers LicenceThis is a full-time one year contract offering a highly competitive renumeration (with the potential to extend).
