About the role We are expanding our team and seeking to recruit dedicated individuals who strive for best practice, are able to communicate well with people of all backgrounds and approach every situation with care and compassion. The Care Coordinator will be responsible for: Building strong connections with clients and their families Assessing client needs and resources for health, welfare and other facilities and services Conducting Client Care Assessments and Care Planning Maintaining accurate records as per the Aged Care Quality Standards and NDIS Practice Standards, Quality and Safeguarding Framework Supporting all clients to meet their goals Supporting and providing feedback to Home Care Workers Re-organising work priorities to meet the needs of a fast-paced environment Must promote the values of the company and prepared to plan supports using a contemporary approach Experienced in assessing and planning care needs Sound understanding of the Aged Care Quality Standards and NDIS Practice Standards, Quality and Safeguarding Framework Organisational, administrative and time management skills All candidates must have the right to work in Australia Registered Nurse or Enrolled Nurse (Desirable not essential) #J-18808-Ljbffr
