ABOUT THE ROLE An exciting opportunity exists for an experienced Care Coordinator to join the Salvos Home Care team in South Australia.
The Care Coordinator is primarily responsible for the ongoing coordination and management of a comprehensive range of services and supports provided to an allocated group of Home Care Package clients.
