Right at Home Sunshine Coast provides high quality in-home care and assistance to our valued clients in the Sunshine Coast and Noosa region's. Our mission to improve the quality of life for those we serve. We provide care to those in aged care, post-operative care and people living with disabilities. Our services help clients remain safe and independent in their own homes.

The Care Coordinator will be responsible for: Building strong connections with clients and their familiesOrganise support to clients with disabilities and monitoring and reporting on their progressAssessing client needs and resources for health, welfare and other facilities and servicesResponding promptly to emails, enquiries and requests from clients, staff and managementMaintaining accurate records as per the Aged Care Quality Standards and NDIS Practice Standards, Quality and Safeguarding FrameworkRe-organising work priorities to meet the needs of a fast-paced environmentSupporting all clients to meet their goalsConducting Client Care Assessments and Care PlanningSupervision and management of Care StaffParticipating on rotation with providing 'On Call' supportExperienced in assessing and planning care needsSound understanding of the Aged Care Quality Standards and NDIS Practice Standards, Quality and Safeguarding FrameworkAll candidates must have the right to work in AustraliaOrganisational, administrative and time management skillsDrivers LicenceAbility to Obtain: National Police CheckWorking with Children CheckNDIS Worker Screening CheckFirst Aid & CPR CertificateMust Promote the Values of the Company and prepared to plan supports using a contemporary approach.
