Description Right at Home is a leading in-home care provider of client-focused, relationship-based care for ageing adults. Our dedicated and friendly staff help with a range of personal and lifestyle needs while providing compassionate care for ageing adults and those living with disabilities.
Right at Home Brisbane North and South supports NDIS clients living with mental and physical disabilities, in addition to supporting aged care clients.
We are looking for an experienced, motivated and goal-focused Care Coordinator to manage and assist in the delivery of services to our clients. This can include arranging the repair of wheelchairs or walkers, organising Lite 'n' Easy meals, supervisory visits, and assisting in booking medical appointments to name a few.
The ideal Care Coordinator would be responsible for:
Managing and coordinating services for NDIS and HCP participants. Engaging in stakeholder meetings with the client's relevant parties. Conducting regular assessments in line with client goals. Building relationships with client stakeholders. Scheduling appointments with client service providers in accordance with their changing needs. Maintaining a positive attitude with clients and staff members in a constantly shifting environment. Assisting other office staff in answering phone calls from clients and care workers. Documenting and maintaining client compliance. Requirements Cert III/IV in relevant Aged Care/Disability qualification with 1 year's experience in a coordinating role, or RN with community nursing background with 1 - 2 years' experience and AHPRA registration, or EN with community nursing background with 1 - 2 years' experience and AHPRA registration. Experience in Aged & Disability care. First Aid/CPR Certificate. Australian Driver's Licence. NDIS Workers Screening Card (P). Blue Card (P). Criminal History Check. About you Experience with Microsoft Word, Excel, and Outlook. Previous admin experience (desired but not essential). Ability to work in a team environment.
