About the role
Our supportive Community Care team based in Bruce are seeking an enthusiastic, professional Care Coordinator to join the team in a Permanent Full Time position.
As a Care Coordinator you will provide complex care coordination in the rostering of services for clients using our Client Management System with the aim of assisting people to live independently in their homes. You will work closely with Case Managers, clients, carers and external services in the provision of appropriate, targeted and quality services to our clients.

Key Responsibilities

To provide complex care coordination to clients across various funding streams
To develop positive working relationships with Case Managers to provide streamlined high quality service delivery which is person centred and reablement focussed
To be responsible for data entry and care coordination activities
To build positive relationships with Support Workers to promote person centred and reablement focussed service delivery
To develop partnerships with clients to ensure proactive solution focused support

What you bring

Current Drivers Licence essential
Certificate IV in Community Services or a related field
Experience in Aged Care, Disability or Community Care
Experience with people with Dementia
Previous experience in care coordination
Knowledge and understanding of person centred service delivery
