An exciting opportunity has become available for a Care Coordinator position at Peace Haven.
About The Role: The Care Coordinator is responsible for coordinating residents' person-centered clinical care in line with policies, procedures and professional standards, and in collaboration with the Care Manager. The primary of purpose of the Care Coordinator is to focus on individual resident needs, ensuring that they have a current documented Care Plan, which meets their needs, and is reviewed and delivered by the multi-disciplinary care team.
Key Duties and Responsibilities Resident Care The Care Coordinator works in collaboration with the Care Manager to ensure effective coordination of resident care, including:
Assists with the admission of new residents to the facility, ensuring that they are assessed Assess the individual needs of residents and develop person-centred care plans in consultation with other healthcare professionals, ensuring that care is tailored to meet their specific needs Collaborate with other healthcare professionals to deliver comprehensive care to residents Communicate effectively with residents' families and health care providers, keeping them informed about their health conditions, care plans, and any significant changes or concerns Maintain accurate and up-to-date resident documentation, including care plans, behaviour charts, medication charts, observations and/or incidents Provide and assist with direct nursing care to residents, including and not limited to assisting with activities of daily living (ADLs), administering and managing medication, catheterisation, conducting observations and monitoring, and wound management Actively participates in case conferences and relevant meetings Complete clinical assessments and reviews at required intervals and escalate as required, including COD status, daily wound care and fall management Undertake audits as required, including the Schedule 8 drug count Assist with pharmacy ordering as required Check alerts, overdue tasks and progress notes to ensure they are completed effectively and to standard Coordinate infection control measures are in place and ongoing risk reduction strategies are being implemented Assist with the investigation of clinical incidents and their close out as required Collate data and prepare information for reports as required Staff Supervision: The Care Coordinator works in collaboration with the Care Manager to ensure care team staff are effectively delivering resident care in line with policies, procedures and professional standards:
Acts as a positive role model, demonstrating best practice professional standards Allocates staff and resources to residents depending on resident needs and delegates appropriate responsibilities to staff Communicates accurate and timely information to staff, so that they can effectively deliver safe, quality care to residents Provides guidance to staff in the planning, implementation and evaluation of care and services to meet resident needs Assist with the orientation of new staff, including the arrangement of buddy shifts, competency assessment and system familiarisation Facilitate the identification of education, training and development needs of staff, and assist with the delivery of education, training and development Duty of Care: The Care Coordinator works in collaboration with the Care Manager to ensure duty of care to residents and staff is met by:
Adhere to policies, procedures and professional standards, including the Nurses Professional Code of Conduct and Standards for Practice Advocate for the rights and wellbeing of residents, ensuring that their dignity of risk is managed appropriately Evaluate clinical care provision against industry best practices, governing codes of practices and legislation Respond promptly and appropriately to resident deterioration and/or incidents Assess situations for risk or potential risk to residents and mitigate risk
Professional Development Be committed to their continued professional development Participate in the performance appraisal process as required Attend all meetings and training as required Identify education, training and development opportunities and raise them with their manager Develop and maintain knowledge of aged care issues, standards and services Essential: Bachelor of Nursing Current Australian Health Practitioner Regulation Agency (AHPRA) registration Nursing experience within a residential aged care facility Experience of working cooperatively with multi-disciplinary care teams Strong clinical assessment, documentation and practice skills Effective communication and interpersonal skills Proficient computer and information management skills, including ability to use Microsoft and clinical software systems. Desirable: An understanding of the aged care accreditation process Working knowledge of quality improvement programs Current Driver's License.
