We're looking for an experienced Home Support Assessor Supervisor to join our Aged Care Assessment team and lead a group of dedicated assessors providing compassionate and tailored care to our clients across Bundaberg, Hervey Bay and Maryborough Hospitals within the Wide Bay Hospital and Health Service.

Your opportunity: To supervise and lead a team providing non-clinical home-based face-to-face assessments of clients referred via the My Aged Care Gateway using the Integrated Assessment Tool (IAT). Under the Single Aged Care Assessment System, Home Support Assessors will work with clients and their representatives to establish goals specific to the client's needs and develop appropriate strategies and actions, including referrals to aged care and non-aged care services. As a supervisor, the position will mentor new staff in their Home Support Assessment role, continue education and quality assurance for Home Support Assessors, and undertake Delegate responsibilities as outlined within the New Aged Care Act. The Single Aged Care Assessment System is a multi-disciplinary team providing assessment services to older Queenslanders in the community. The assessment system aims to simplify and improve access to aged care services. The assessment framework is designed to streamline the assessment process and enhance the overall experience for our older Queenslanders. Several Permanent Full Time Positions (76 hours p.f.)
Our offer: You will receive a competitive remuneration package value up to $114,177 p.a., comprising salary between $90,832.50 - $100,071 p.a., employer contribution to superannuation (up to 12.75%) and annual leave loading (17.5%) (AO4) (Several Permanent Full Time, 76 hours p.f.).
