An exciting opportunity to apply for a Clinical Nurse Consultant (CNC) position has arisen at the Brown Nurses, a community-based nursing organisation located in inner city Glebe. Our CNC role provides clinical leadership for the broader team.
The Brown Nurses provide high quality care to vulnerable people experiencing isolation, who are at risk of homelessness and have been marginalised from accessing health and welfare services whilst experiencing chronic-complex health conditions. Our unique service aims to support our clients to actively engage in healthcare and access the health services of their choice. We partner with local services to provide assessment, treatment, education, support and referral utilising a person-centred and trauma-informed approach.
Our Clinical Nurse Consultant is a member of a small but dedicated team which provides expert nursing care to our clients living in the City of Sydney Local Government Area. In collaboration with our clients and the wider multidisciplinary healthcare team, our nurses assess, plan, implement and evaluate nursing care, to achieve client goals and optimal outcomes. This includes appropriate referral to, and consultation with, other relevant health professionals, service providers, community and support services.
