Younger people are defined as people less than 65 years, or Aboriginal and Torres Strait Islander people aged less than 50 years. The Program assists with a range of daily activities including personal care, dressing, preparing meals, house cleaning, property maintenances, community access and using public transport.
As an Assessment and Facilitation Officer, you will complete holistic assessments of need to support the development of goal directed support plans. The role will utilitse a strengths-based approach and work in partnership with other service providers, agencies and professionals to ensure that individual clients needs can be met. The Assessment and Facilitation Officer will also lead and participate in community development and social inclusion activities.

About you To be successful in your application you will meet the person specification in the position description, including having a tertiary qualification in Disability, Social Work, Allied Health or a related discipline.
You will also have:
Previous experience in the provision of care to elderly, younger people or people with disabilities Demonstrated experience in performing community and/or health assessments using a strengths-based approach Excellent engagement, care planning and assessment skills Ability to work collaboratively with other services, agencies and professionals to achieve optimal client outcomes Current first aid and CPR certificates Click here to view the position description.
