Make a difference by supporting clients to achieve their goals and live independently in their own homes. We are looking for a confident and passionate Client Liaison/Team Leader to join our team! This is a dual role working half of the time as a Team Leader supporting our Home Care Workers & Cleaners and 50% of the time as a Client Liaison based at our Hervey Bay service.
A typical day in your new role: Provide supervision, leadership & guidance to Home Care and Support Service workers Assist with the orientation of new staff Undertake assessments, support planning and package care coordination Undertake case management activities Supporting the clients with their budgets What you need to apply: Certificate IV in Home and Community Care or equivalent Current National Police Certificate - or willingness to obtain Current Drivers Licence and own reliable vehicle To be successful you will also bring: Proven understanding of assessments, packaged care and case management Ability to engage clients and work effectively with them to achieve identified goals Support and guidance to clients with budget management and financial sustainability Experience in monitoring and assessing services provided to ensure they are following service models and guidelines Exceptional communication skills - both written and verbal - and the ability to relate to a wide range of people Ability to advocate strongly for your clients and the services they require A sound understanding of the community care system and the people who utilise this service Diplomacy and negotiation skills; people management capabilities Why Join Anglicare?
