iLA is pleased to announce our successful appointment as a provider of Residential Aged Care (RAC) Funding Assessments as a part of the Single Aged Care Assessment Service across the metropolitan region of Western Australia.
As a growing for-profit organisation, iLA is seeking an Aged Care Assessor (Non-Clinical) to join our assessment team.
THE ROLE
We are seeking a detail-oriented and experienced Non-Clinical Assessor to join our team.
In this role, you will be responsible for evaluating non-clinical assessments underpinned by a reablement and wellness philosophy to support older people in remaining living independently and engaged in their community.
Within the role, you will be required to undertake holistic non-clinical assessments of older Australians living in the community and develop support plans which take into account the individual's health, physical, social, and functional needs and goals, and available services.
KEY RESPONSIBILITIES:
Perform home support assessments for older adults using the Integrated Assessment Tool (IAT) to support independent living and determine eligibility for entry-level services.
Review, recommend, and develop support plans through home visits or phone/video conferences with clients, carers, and family members.
Conduct culturally sensitive assessments for groups such as Aboriginal and Torres Strait Islander individuals, those from CALD backgrounds, the LGBTIQ+ community, or those at risk of homelessness.
Collaborate with clients, families, and healthcare professionals to tailor support plans and develop innovative support options.
ABOUT YOU
The ideal candidate is a diligent, empathetic, and driven individual who is motivated by their passion to assist and support the vulnerable members of the community.
Holds a minimum Cert IV qualification in aged care/community services or a related field or has demonstrated experience working in a related area.
Has demonstrated experience working in the aged or community care sector in an assessment-based role.
Applies person-centred assessment skills which demonstrate a passion and authenticity to empower the consumer.
Has the ability to manage time effectively and work autonomously.
Has experience utilising and implementing reablement strategies with consumers.
Possesses advanced IT skills with previous experience working across multiple platforms (Microsoft Office Suite).
Maintains a current unrestricted Australian driver's licence.
