Managing Clinical Consultant - Aged Care Age Care Directions (ACD) specialises in Aged Care Planning, Advisory and Placement Services.
Established in 2005, we are driven by a passion to assist older people and their families find the best possible care and accommodation solutions, specific to their needs and preferences.
The Role:
We are seeking a Nurse with aged care experience for a full-time role, based in our Fullarton office, to commence in early 2025.
The successful applicant will have demonstrated knowledge of residential aged care, home care packages, and retirement accommodation along with business development experience.
The key responsibilities of this role include:
Leading a small team to drive growth in the community aged care sector.
Conducting clinical client assessments.
Preparing care need summaries and/or care plans.
Identifying clients' requirements for placements/services via person-centred reports.
Convening meetings with the client/s and/or their advocates to outline the process for accessing placements/services.
Processing applications for residential aged care facilities.
Liaising with colleagues to ensure the best client outcome.
Building relationships with all stakeholders to promote ACD.
You will need to demonstrate:
Current AHPRA registration.
Experience in the aged care sector.
Proven business development experience and financial acumen.
Exceptional written and verbal communication skills.
Current knowledge of AN-ACC assessment and funding.
Current Driver's Licence.
Current Police Clearance and NDIS Worker Screening Check.
Computer literacy, including MS Office suite and CRMs.
