Advanced Nurse Practitioner - Moorlands Rural PCNTo work as an autonomous practitioner, as part of the multidisciplinary team, delivering care within their scope of practice to the patient population. Manage patients presenting with a range of acute and chronic medical conditions, providing subject matter expert advice to patients.
The advanced nurse practitioner will be responsible for a number of clinical areas such as health promotion and minor illness, as well as supporting the management team in reviewing clinical policy and procedure.
Main duties of the jobWe are looking for an Advanced Nurse Practitioner who is passionate about providing excellent care, compassionate, and a real team player. You will be working alongside an approachable and supportive GP Partner every session.
About usAlthough you will be part of the Moorlands Rural PCN team, you will mainly be based at Werrington Village Surgery. We are a caring, friendly, and supportive practice looking to recruit a dedicated team member to help maintain our high standard of patient care. We are a forward-thinking training practice caring for 8,500 patients. The practice has a village feel and is supported by an active Patient Group.
Job responsibilitiesPrimary Responsibilities Manage patients presenting with a range of acute and chronic medical conditions, providing subject matter expert advice to patients.Develop, implement and embed health promotion and well-being programmes.Implement and evaluate individual specialized treatment plans for chronic disease patients.Identify, manage and support patients at risk of developing long-term conditions, preventing adverse effects on their health.Provide advanced, specialist nursing care to patients as required in accordance with clinical based evidence, NICE and the NSF.Request pathology services as necessary.Process and interpret pathology and other test results as required.Provide chronic disease clinics, delivering patient care as necessary, referring patients to secondary/specialist care as required.Maintain accurate clinical records in conjunction with extant legislation.Support the team in dealing with clinical emergencies.Recognize, assess and refer patients presenting with mental health needs.Extended and supplementary prescriber, adhering to extant guidance.Support patients in the use of their prescribed medicines or over-the-counter medicines (within own scope of practice), reviewing annually as required.Contribute to practice targets (QOF etc.), complying with local and regional guidance.Delegate clinical responsibilities appropriately (ensuring safe practice and the task is within the scope of practice of the individual).Person Specification Personal QualitiesPolite and confidentFlexible and cooperativeMotivated, forward thinkerProblem solverHigh levels of integrity and loyaltySensitive and empatheticAbility to work under pressureEffective communication skillsCommitment to ongoing professional developmentPunctual and committed to supporting the team effortQualificationsRegistered NursePost graduate diploma or degree (Advanced Practice Qualification)Qualified Nurse PrescriberTeaching QualificationExperienceExperience of working in a primary care environmentExperience of prescribing and undertaking medication reviewsExperience of working as a practice nurse or community nurseSkillsExcellent communication skills (written and oral)Competent in the use of Office and OutlookEffective time managementAbility to work as a team member and autonomouslyGood interpersonal skillsOther requirementsFlexibility to work outside of core office hoursOccupational Health ClearanceDisclosure and Barring Service CheckThis post is subject to the Rehabilitation of Offenders Act (Exceptions Order) 1975 and as such it will be necessary for a submission for Disclosure to be made to the Disclosure and Barring Service (formerly known as CRB) to check for any previous criminal convictions.
£47,000 to £56,000 a year dependent on experience.
