About the Opportunity We're searching for a talented professional to step into the role of Access and Community Brokerage Manager! In this pivotal position, you will provide clinical expertise for individual patients while leading initiatives within our innovative CO-ACT Unit.
In this role, you will:
Deliver and oversee expert clinical care tailored to individual patients. Lead and inspire the Access and Community Brokerage Multidisciplinary team. Coordinate and spearhead projects or programmes that enhance patient, client, and service outcomes. Exercise significant autonomy in clinical decision-making. Build and nurture strong relationships with both internal and external stakeholders at CALHN, ensuring a seamless patient journey into and out of the CO-ACT program. Take accountability for your team's outcomes, ensuring alignment with policy and quality standards. We Are Looking For:
