Your role   Australian Unity's Home Health platform is proud to partner with Ramsay Connect on the Diabetes Using Our Strengths Service (DUOSS) pilot research program.
This innovative program combines face-to-face and virtual care to empower Aboriginal and Torres Strait Islander people to manage their type 2 diabetes.
Through remote monitoring, medication management, health education, and group yarn ups, DUOSS provides holistic support in a culturally respectful setting.
As our Aboriginal Peer Learning Officer , you'll be a key player in this journey, supporting Aboriginal and Torres Strait Islander patients to engage in DUOSS and working closely with our clinical team to deliver compassionate, meaningful care.
This is a full-time (will consider part-time) fixed term role of 12 months, with travel to Bundjalung Country required.
This role requires living on Bundjalung Country within a commutable distance to Tweed Heads, NSW.
About you   You will engage with team members and customers virtually (phone, e-mail, video) or face to face, using your lived experience with diabetes to provide holistic support to patients enrolled in the service.
As the Aboriginal Peer Learning Officer, you will: Provide respectful, culturally safe support to Aboriginal and Torres Strait Islander patients living with diabetes Facilitate regular yarns by phone, videocall, or face-to-face in patients' homes Organise activities that support patients' health and well-being Liaise with clinical staff about any concerns and patient needs Help patients with technology for health monitoring and communication Work from the Australian Unity Tweed Heads office at least three days a week Your expertise and qualifications will include:   A demonstrated ability to communicate effectively and sensitively within a diverse team A strong understanding of cultural safety, with an ability to support Aboriginal and Torres Strait Islander patients compassionately Willingness to travel for face-to-face appointments on Bundjalung Country Confidence in using and troubleshooting patient apps and health monitoring devices Knowledge of privacy and confidentiality practices in a digital health setting Desirable: Experience working in community or virtual care To perform this role, it is essential that the person is an Aboriginal and/or Torres Strait Islander person.
It is therefore a genuine occupational requirement under section 25 of the Anti-Discrimination Act 1991.
