(Pro Rata) ASO5 | Part-time, 26.25 h/pw – Term up to June 2025 Join our Aboriginal Health Team to support child and maternal health for this vibrant community.About the role: Reporting to the Principal Project Officer, this role will be involved in testing continuity of care programs across Southern Metropolitan Adelaide.
Duties for the role will include: Undertaking care coordination for a small caseload across SALHN to support continuity of care across their health care journey, including support and advice for other members of the health care team.Identifying and accessing appropriate referral pathways based on a person's needs and goals of care.Liaising with families, carers, health practitioners, health care workers and others who may be involved in the provision of services to a mother of an Aboriginal baby or an Aboriginal child aged 0-4 years.Contributing to improved cultural competence across SALHN and developing enhanced pathways and links with community health and wellbeing services.
About you: Demonstrated ability to liaise, consult and engage in a culturally sensitive manner with the Aboriginal community, individuals and families with proven commitment to improving outcomes for Aboriginal people.Demonstrated high level of interpersonal, written and verbal communication skills, including the ability to provide analytical commentaries and consultancy services to staff at all levels.Demonstrated negotiation and consultation skills with proven ability to engage and develop effective partnerships with a range of stakeholders.
