In this role you will provide quality client-centred support, to young people aged 15-24 years of age who are experiencing homelessness or at risk of homelessness and requiring crisis accommodationYou will successfullyEnsure Support is provided to young people in line with Youth Services Model of care and associated FrameworksBuild effective relationships with clients that encourage self-determination and participationEnsure case management support is provided to clients in accordance with TSA case management models and standards of practiceIdentify and raise risks to clients and staff to the attention of the line managerReport and record incidents/hazards via online management system timely and accuratelyIdentify service gaps and provide recommendations for service delivery improvements to Team Leader / Program ManagerThese Part time roles are located in Karratha, WA.
