This is a 7 month fixed term, part-time (0.3 FTE) position commencing Term 4, 2025 with possible extension and/or permanencyWarnbro Community High School (CHS) is located 50 km south of Perth, nestled in the beachside communities of Warnbro and Port Kennedy.

We offer unique and effective learning programs grounded in evidence-based strategies that ensure progress for all students.Find out more about our school by visiting or The RoleThe Defence Secondary Transition Mentor will be responsible for maintaining and enhancing systems that provide a responsive and effective youth support service for students of ADF members and family members.
They should maintain and establish professional links between schools and various public and private sector organisations, develop, plan and implement youth work interventions, and provide a supportive link between the student, the family, the schools and other appropriate agencies in the community.
The successful applicant will ideally actively participate in multi-disciplinary team meetings and case conferences within the school, including the provision of consultancy advice.
They will ideally research current issues for students from mobile ADF families, providing written reports outlining findings.
The Defence Transition Mentor may be responsible for day-to-day management matters, as well as responding to complex demands during critical situations.
