Job DescriptionThe position provides direct case management support to all young people accommodated within the crisis refuge, aged between 12-19 who are homeless or at risk of homelessness, with the aim to build on their skills and capacity to work towards independent living or family restoration.
This position provides support to the Refuge Manager in regard to program delivery and guides a team of youth workers to ensure a consistent, practical and supportive environment for all young people within the refuge program.
This position, alongside the Refuge Manager, will support the refuge intake and assess the suitability of a young person, identifying any risks that could place staff or other young people at risk and developing and implementing strategies to, where possible, reduce this risk. This includes appropriate documentation around assessments and case noting.
The program has a large focus on capacity building and supporting young people to engage in education/employment and work towards their identified case plan goals while linking them with the appropriate services to achieve wrap-around supports.
To support the young people to work towards their case plan goals, this role will liaise with external services to link in, advocate, and complete referrals. The position will provide flexible, specialist services that accord with principles of self-determination and meet the needs of this vulnerable target group. The position would be required to work as part of a large team and also, at times, in isolation.
The position would function under the supervision of the Refuge Manager.
Selection Criteria:Support all intake assessment tasks, including assessing suitability of referrals, administration tasks, obtaining information from other services involved with the young person (such as school, police, youth justice) and documentation of work.Undertake initial needs assessment and risk assessment for young people upon intake and continue to conduct regular and ongoing assessments of the young people's needs.Develop individual case plans in collaboration with the young person and residential worker if relevant. This includes providing direct financial assistance via brokerage to young people with a case management plan.Refer and advocate for young people, ensuring that their needs are accurately understood and met (where possible) and that the young people are involved in any decision-making processes that affect their lives.Ensure the accurate recording of all statutory and information requirements for client records and case planning, including updating CIMs reporting, case note sharing, records management, statistical and other data collection. This includes:Details of requests for accommodation.Service intake and assessments forms.CIMS data.All relevant records pertaining to the case management program.House meeting books.Staff meeting books.Any relevant statistics required by management and staff.Correspondence.General filing.Support and assist residents/young people to appropriate educational or training pathways, including providing an employment skills program, such as resume writing, interview presentation, and interview skills, both in-house and with external resourcing.In conjunction with the Refuge Manager, oversee the delivery of service by the Residential Workers to ensure it meets the needs of the clients, including supporting and assisting residents/young people in developing independent living skills so they can exit safely out of homelessness.Arrange and participate in leisure activities with residents, including school holiday activities.Ensure that all services are provided in a fair and equitable manner.Ensure the service is culturally sensitive and takes into consideration all needs of the clients as much as possible.Provide both on-site and outreach/mobile response, including visits to home, schools, other education facilities, employment services, prospective employers, and relevant government agencies (Centrelink).Maintain the strictest confidence of information pertaining to refuge residents and staff.Where relevant and in consultation with the Refuge Manager and HR, assist with the recruitment process, which would include culling candidates and being part of the interview process.Support the Refuge Manager with refuge tasks, such as timesheets, reconciliations, rostering, and property maintenance.Be part of the on-call after-hours roster and provide advice to staff regarding complex cases and crisis situations.Establish and maintain collaborative co-case management partnerships with appropriate SHS and mainstream youth services to achieve quality outcomes for young people and their families.Desired Skills and ExperienceFormal qualifications or higher in community sector or equivalent experience.Minimum 2 years' frontline casework experience and demonstrated extensive experience working with youth at risk. Prior experience in a refuge setting is desirable.Demonstrated ability to conduct thorough assessment and intake processes and make appropriate recommendations.Demonstrated experience in all aspects of case management and case plans, including case noting, ability to meet deadlines, manage competing priorities, be flexible, and work with minimal supervision.Demonstrated excellence in working with Culturally and Linguistically Diverse people, families, and organisations and planning, delivering, and evaluating services in a culturally sensitive manner.Well-developed interpersonal, mediation, and conflict resolution skills.Demonstrated understanding of child protection issues with the capacity to identify levels of risk requiring statutory child protection intervention.Demonstrated knowledge of and experience in the development and implementation of data collection tools and data evaluation processes, including the ability to generate comprehensive reports and manage brokerage budgets in a complete, timely, and accurate manner.Sound interpersonal, communication skills and high level of computer literacy.Possess a valid NSW Working with Children Check and Australian Federal Police Check.
