Are you passionate about making a real difference in the lives of young people at risk of homelessness and their families? Join our dynamic team as a Youth Practitioner, where you'll play a crucial role in providing support, guidance, and hope to vulnerable youth and their families.
In this exciting and rewarding role, you'll offer personalized case management, family support, and lead group work programs to help prevent homelessness and build brighter futures. You'll work closely with young people to understand their unique needs, create tailored action plans, and connect them with the services and resources they need to thrive.
You'll be the go-to person for providing essential information, advice, and referrals, while also building strong, supportive relationships with families to help strengthen their foundations. You'll be part of a passionate, collaborative team committed to creating lasting change and empowering young people to overcome challenges and reach their potential.
Your key responsibilities will be to:
Provide intensive ongoing person-centered case managementAssist clients through brief therapeutic intervention, case management, advice, information, and referralConduct group case management sessions for clients where necessary and appropriateAssist Program Manager and other employees in addressing complex cases or emergency situationsComplete a range of administrative duties for the efficient running of the serviceBuild and maintain positive internal and external relationshipsRequirements for Success Hold a relevant degree and at least 1 year of experience in a similar roleDemonstrated professional approach to the role including the ability to maintain professional boundariesDemonstrated ability to provide specialised therapeutic interventions and manage a caseload of complex clientsDemonstrated ability to apply basic counselling and mediation frameworks and interventions to young people and their familiesA current and valid Drivers' licenceBefore starting work with us, you will need to undertake a national police history check, qualifications, and referee checks, and have a clearance to work with children/vulnerable people.
