Your Opportunity The Youth and Family Worker role in Moreton Bay supports young people and their families through the Assisting Students and Parents (ASAP) program, providing flexible case management tailored to individual needs.
Services are delivered in schools, community settings, and family homes, with a focus on maintaining engagement in education or vocational settings, fostering improved family relationships, and encouraging active community involvement.
The role is integral to the success of programs like ASAP, which supports young people at risk of disengaging from education, and linking clients with accommodation options, counselling, mediation, and other vital supports.

Your Key Responsibilities Provide person-centred case management support for clients in line with MA's National Case Management Approach, including creating individualised, goal-oriented support plans that are SMART, and conducting regular reviews with clients and supervisors.
Work collaboratively with high schools and wellbeing teams to connect with referred students, assess suitability for support, and facilitate improved family relationships, school engagement, and participation in education or vocational settings.
Respond to client referrals from internal and external support services, conducting thorough assessments and completing intake processes, including providing information on service rights, responsibilities, and consent.
Develop professional relationships with schools and external stakeholders, collaborating with school student support teams, other service providers, and government agencies to facilitate seamless client support and referrals.
Actively advocate for clients with other services and assist them in accessing supports, including attending appointments and providing transport if required, to promote independent engagement.
Build culturally safe relationships with Aboriginal and Torres Strait Islander communities and relevant services to enhance engagement and deliver culturally informed support.
Create and maintain accurate case management files and records in line with Mission Australia protocols, ensuring completion of internal and external reports, impact measurement, and quality program activities.
Requirements for Success With a significant background in Community Services and/or Mental Health sectors, you will be confident in your values based approach and assessment processes.
Other requirements include: Relevant tertiary qualifications in Community Services, Social Work, Behavioural Sciences, or a related human services field, with at least 2-3 years of experience.
Demonstrated ability to support young people experiencing complex challenges, including engaging with parents and/or carers, and providing specialised or complex therapeutic interventions.
Strong crisis management, case management, referral, and advocacy skills, with knowledge and understanding of Child Protection legislation and child safety concerns.
Effective time management and organisational skills to manage a caseload, meet deadlines, and balance additional program activities as required.
Well-developed written and verbal communication skills, including the ability to prepare clear, concise case notes and present information effectively to diverse audiences.
Strong listening and engagement skills, with the ability to build and maintain positive relationships with clients, colleagues, and external stakeholders.
A professional and resilient approach to the role, maintaining professional boundaries, self-awareness, and personal wellness, while contributing positively to team dynamics and work practices.
Must hold current driver licence with no restrictions Before starting work with us, you will need to undertake a national police history check, qualifications, and referee checks, and have a clearance to work with children/vulnerable people.
