Orange Aboriginal Medical Service (OAMS) is making a difference to the health of Aboriginal families and the wider community of Central Western NSW. Commencing in 2005 with only a handful of staff members, OAMS has now expanded across 5 locations, providing a wide range of services to the whole community.

The yiriyirimbang Case Manager plays a vital role in providing Permanency Support Planning (PSP) for Aboriginal children and young people living in Out of Home Care (OOHC), their families and carers.
The Case Manager will be responsible for providing all aspects of case management support for Aboriginal young people, their families and foster/kinship carers.
You will also need to work in collaboration with families and community stakeholders to assess and work towards permanency for each young person. This will include helping them to build connections in the community as well as assisting them to independent living.
To be successful in this role you will need to demonstrate the following:
Relevant Diploma in welfare, community services or related field with experience in working with children and familiesAbility to work rostered on-call shifts and travel requirementsDemonstrated adept knowledge, skills and experience working with Aboriginal children, young people in the OOHC/PSP settingPractical experience of working as a multi-disciplinary teamAdvanced level of oral and written communication skills, including record keepingKnowledge of and Commitment to privacy and confidentialityCommitment to participate and complete all mandatory training and formal qualificationsAdditional Factors Possession of a current 'C' Class Open Driver's Licence valid in NSW is requiredCurrent First Aid and CPR CertificateCurrent NSW Working with Children's CheckCurrent Criminal History checkProof of vaccination must be provided upon acceptance of appointment #J-18808-Ljbffr
