About the role

In the role of Wellness and Respite Coordinator you will drive community engagement and day centre program expansion in our recently renovated day centre, Chantal Cottage.
Success in this role looks like coordinating and facilitating friendly and engaging environments where seniors living in the community come for connectedness, stimulation and carer respite to maintain quality of life.
Day to day this looks like:

Ongoing coordination and management of the program calendar of events, providing opportunity for clients to participate in activities of choice.
Ensuring weekly programs are delivered with assistance of staff and volunteers, responding to client requests, choices and feedback.
Promotion, administration and co-ordination of a range of health, wellness and social programs for community living clients.
Implementing plans to ensure KPIs are met with regular reporting such as attendance tracking, client feedback and care goals.
Involving clients and carers in developing individual care plans that are person centred and reablement focused to maintain quality of life.
Developing and maintaining strong relationships and referrals within the community.
Actively promoting the wellness/respite center within the local community, directly or supporting scheduled events.
Assisting clients and potential clients to navigate aged care needs and supports whilst promoting their health and wellbeing.
Developing and maintaining strong relationships with client carers and representatives, including providing warm welcome and regular feedback.
Providing direction and supervision to Community Workers and Volunteers during the services.
Ensuring client safety during programs, including conducting environmental and activity risk assessments and utilising qualified and/or trained instructors.

To be successful in this role you'll need:

Qualifications in leisure and lifestyle, health promotion, therapeutic recreation, allied health, social science or similar.
Preferable experience in facilitating community events and ongoing group program management.
Ability to drive and promote day centre program growth within the community.
Experience in facilitating and managing initiatives to help improve health, social connections, and quality of life of people.
Experience working in a community setting and demonstrated knowledge (or willingness to learn) the aged and disability service system in the community health sector.
Willingness to apply for a Police Check and NDIS clearance.
