Wellbeing Case Manager (Seymour)

Join Junction Support Services as a Wellbeing Case Manager at Seymour Flexible Learning Centre (SFLC).
In this role, you will provide individualized case management and support to students, focusing on their emotional wellbeing and offering crisis response when needed.
You'll work closely with teaching staff and external services to create a supportive and inclusive environment, helping students overcome challenges and build resilience.
Your key responsibilities will include: Providing one-on-one support to students Promoting health initiatives such as harm minimization and psychosocial education Developing and maintaining partnerships with relevant services Assisting in making referrals to internal and external support organizations Contributing to the overall wellbeing of the school community
