What you'll be doing
The Wellbeing Health In-reach Nurse Coordinator (WHIN) facilitates identification and coordination of the response to health, wellbeing, and social needs of school-aged children and young people. This role works across community settings and is based in schools across three to four schools in the local region. The WHIN Nurse works in an early intervention, case management, and coordination model in partnership with Health and Education, and will support the implementation and evaluation of this project until June 2024.
Selection Criteria Current Registration with the Australian Health Practitioners Regulation Agency (AHPRA) as a Registered Nurse and relevant post-registration qualifications in the specialty field (child and family health, youth health, counselling/psychosocial care) with a minimum of three years' experience working in the clinical specialty OR current registration with AHPRA as a Registered Nurse and a minimum of three years' experience working in the clinical specialty with a commitment to completing post-registration qualifications in the specialty field within two years of employment. Demonstrated competency and knowledge of the complexities and challenges involved in providing nursing care in the context of a child and family health and youth-based care team. Demonstrated ability to exercise extended autonomy in clinical decision-making capabilities and leadership in the development of nursing specialty practice advancement. Demonstrated effective clinical communication through the use of information technology, written and verbal means, and advanced interpersonal skills with the capacity to interact with others in a personable and professional manner. Demonstrated knowledge and application of evidence-based practice and critical thinking to advocate for and implement individualised person and family-centred care at an extended role level within a child and family health, youth health, or psychosocial focused service model, with a commitment to quality improvement. Demonstrated ability to supervise, delegate, and negotiate with professionals across health, education, and community services to deliver quality health care with a willingness to show leadership and embrace change in the coordination and delivery of care. Evidence of participation in own professional development, a commitment to maintaining competence and enhancement of skills, and willingness to apply and share professional expertise as a resource person and mentor others within the specialty. Evidence of a current unrestricted drivers' licence and the ability to maintain. Ability and willingness to drive/travel as the role requires. This is a Category 'A' position. All new employees must agree to comply with the requirements outlined in the relevant NSW Health policy directive. OASV requirements also include proof of vaccination against influenza, which is compulsory for NSW Staff that enter any Residential Aged Care Facility (RACF) or who work in NSW Health owned and operated RACFs.
