War Memorial Hospital (WMH) is a 3rd Schedule Aged Rehabilitation hospital, situated in Waverley, owned and operated by Uniting and part of the South Eastern Sydney Local Health District (SESLHD). Services are comprised of 24-hour inpatient care, Monday to Friday outpatient departments, iREAP Day Rehabilitation program, several specialist community teams inclusive of Transitional Aged Care, Aged Care Assessment Team, Geriatric Flying Squad, Young Onset Dementia, and day centre program, 24-hour accommodation service for regional guests and the full range of hospital support services. The Geriatric Flying Squad provides advanced clinical assessment, investigation and develop management plans for older people in the community and in residential aged care facilities over 7 days per week. This service aims to maintain the health and independence of older people living in residential care and the community by providing rapid access to acute aged care nursing for older people experiencing a decline or acute changes in their condition whilst remaining in their RACF or in their home. This is via the provision of a multi-disciplinary geriatric assessment and short term case management. The aim of the GFS is to (where appropriate) implement hospital avoidance strategies who are at high risk of emergency department presentation or readmission to hospital.
An exciting opportunity has arisen for a suitably skilled, dynamic and aspiring clinician to fill a permanent full time Clinical Nurse Specialist 2 (CNS2) position within the team. The CNS2 is an integral member of the Geriatric Flying Squad (GFS), co-ordinating and delivering high quality nursing care to patients/clients in our community and RACF's. As a member of the multidisciplinary team, the CNS2 undertakes comprehensive assessment, care planning and co-ordination of treatment for frail older people in a sub-acute community environment.
This has proven to be an effective strategy for keeping older people well in their usual residence, reducing avoidable hospital presentations and/or admissions and supporting the older person to remain in a familiar environment. Working well within a multidisciplinary team of nursing staff, allied health and medical professionals is essential.
The role involves working collaboratively with and under the direct supervision of the GFS Head of Department and Director of Nursing to assist in the provision of a service that is comprehensive, accessible, high quality and person-focused. The successful applicant will be provided with a mentored orientation programme by the GFS HOD and be well supported by the GFS Clinical Nurse Consultant, Nurse Practitioners, Staff Specialist Geriatricians, Allied health team and WMH Executive.
The purpose of the GFS CNS 2 role is:

To provide multi-disciplinary comprehensive geriatric assessment and case management of community and residential aged care clients as part of the Geriatric Flying Squad.
To provide accurate and time specific triage of referrals, facilitate evidence-based interventions, prevent avoidable hospitalisations and access appropriate referral pathways for people experiencing a subacute functional decline in the community.
To maintain a high level of communication with other members of the multi-disciplinary aged care teams and with patients and their carers.
Set and maintain high standards of patient care for the WMH Geriatric Flying Squad and across geriatric rehabilitation services.
Provide expert clinical consultancy service to support nurses and other staff working in acute, sub-acute and domiciliary settings in rehabilitation within the philosophy of person-centred care.
To ensure maximum possible integration of patient care occurs across all aspects of service delivery.
To monitor and manage the effectiveness of multidisciplinary care provided.
Plan appropriate strategies to introduce new procedures and clinical guidelines in response to evidence based clinical practice.
Identify key learning needs (including knowledge, skills and attitude) of nurses and other health professionals and participate in formal and informal dissemination of education programs.

This position is a fulltime permanent role. The working days are Monday to Friday, 8am to 4:30pm.
Selection Criteria:

Registered Nurse - List A with current Registration with Australian Health Practitioner Regulation Agency (AHPRA).
Relevant post registration qualifications in aged care, community or Rehabilitation and at least three years post-registration in the aged care, community or rehabilitation field.
Excellent interpersonal and communication skills and the ability to coordinate case management of patients with complex needs.
Exercises extended autonomy of decision making and appropriate escalation as required.
Demonstrated ability to work effectively within a multi-disciplinary team.
Demonstrated experience with clinical documentation systems and Microsoft Office suite with demonstrated highly developed computer skills.
Demonstrated knowledge of Quality Improvement principles and awareness of community service requirements relating to accreditation and WHS processes.
Unrestricted Class C driver's License.
