The Youth Homelessness team delivers early intervention transitional support to young people and couples aged 15-25, and their young families who are homeless or at risk of experiencing homelessness.
As a Youth Case Manager, you will provide engaging case management support to clients from a diverse range of backgrounds, helping them to break the cycle of homelessness.
The role will assist clients to identify and address their individual support needs that contributed to their homelessness.
Working as part of a committed team, the Youth Case Manager will respond to referrals in a timely manner, engage young people through outreach, and support them to access relevant specialist services and programs.

Your skills and experience You will meet the person specification in the position description, including having a Diploma/Advanced Diploma/Associate Degree in Community Studies (Welfare), Social Work or a related discipline.
You will also have: Demonstrated case management experience within the homelessness sector Previous experience working with young people who present with complex needs and challenging behaviours A client focused approach, with the ability to prioritise the individual needs of each client Strong communication, organisation and problem solving skills How to apply Click APPLY to submit your interest.
