The role
As Triage and Assessment Delegate you will play a pivotal role in the newly established Single Aged Care Assessment team at Suncare.
You will be responsible for validating the initial assessment and prioritisation of aged care customer's needs to ensure appropriate and timely access to services and resources.
This role is offered as a permanent, full-time opportunity (Monday to Friday).
Given our number of Suncare offices, including a head office in Maroochydore, there is flexibility with your office location.

As an accountable party in providing services under the Aged Care Act, you will be mindful and act with due diligence in the discharge of your duties.
Reporting to the Team Leader Triage and Delegate Services, these duties will include:
Performing Triage after the acceptance of a referral and prior to conducting an Integrated Assessment Tool (IAT) assessment, validate the appropriateness of an assessment referral received.
Completing Triage questions on the My Aged Care System, to determine the immediate needs of customers seeking aged care services.
Ensuring that assessments have been conducted without errors, contradiction, or omissions before approving the care.
If there are gaps in the evidence, seek more information before making a decision and finalising the delegation process.
Recording decisions and a statement of reason for the decisions, along with supporting evidence.
Keep accurate relevant notes on the customer file on the MAC portal.
Assessing the urgency and complexity of clients' needs to prioritise them for appropriate services and interventions including but not limited to, granting of extension for transition care and residential respite care.
Gathering relevant information from customers, their families, and healthcare providers and developing a thorough understanding of individual circumstances.
Developing actionable recommendations for service allocation based on triage findings.
Maintaining a good knowledge and understanding of the Aged Care Act.
What you will bring to our team:
You will possess a positive attitude for working in the aged care industry.
You will also bring to our team:
Tertiary qualification in a health-related discipline.
Current unrestricted AHPRA registration or part of, or eligible to be part of, a relevant professional association.
Good knowledge and understanding of the Aged Care Act.
Demonstrated ability to work with and embrace current and new technologies.
Adaptability and flexibility to be able to respond to changing task requirements in an enthusiastic and positive way.
The ability to work autonomously and as part of a team.
Willingness to use own vehicle for work purposes, ensuring vehicle is reliable, appropriately registered and insured.
An understanding of the impact of aging or disability, along with experience of triage and working with My Aged Care Portal, would be advantageous.
