About the role The Transition to Adulthood team at CofC support young people who are transitioning from out of home care.
Our teams are funded by the Department of Child Safety, Seniors and Disability Services (DCSSDS) through the Extended Post Care Support and Next Step Plus initiatives.
The Transition to Adulthood team focuses on ensuring young people are adequately prepared to achieve positive long-term outcomes into adulthood.
Our team works with a range of stakeholders to support young people to develop skills and access resources to plan for their future.
We are looking for someone with drive and initiative who can work as a team and autonomously.
Our Transition to Adulthood programs are evidence-informed, structured, and responsive to the uniqueness of individuals.
Young people leaving care need support with navigating systems, building a safe and supportive community around themselves, finding appropriate housing, accessing mental health and allied health services, engaging with training and employment providers, and that's just the tip of the iceberg.
Our overarching goal is to set young people up for success, without creating reliance on paid support services to navigate through life.
About you Are you passionate about working with young people to enable them to reach their potential and build their community network?
We're looking for a candidate with backgrounds in care coordination who are passionate about supporting young people to achieve their dreams.
You understand the influence and lasting positive impact that a mentorship role can have on a young person's life, you are values driven and have a learning mind set.
Your skills knowledge and qualifications include: You will: Deliver effective coaching and care coordination support and provide advocacy for young people transitioning from out of home care Work collaboratively alongside key stakeholders such as natural mentors and supporters, Child Safety, health providers, and community support services to create a positive experience for young people Provide mentoring to assist young people to identify their goals, work out a way to achieve them, and get creative to overcome any barriers Provide young people with practical support around housing, employment, health including mental health, and wellbeing to establish a firm foundation for growth Work sensitively with Aboriginal and Torres Strait Islander people and communities to ensure culturally inclusive service provision and achieve sustainable outcomes within a culturally safe environment.
To be suitable for this position, your qualifications for the role will consist of: Degree qualification in Social Work, Psychology, Social Sciences or a related discipline Two years' experience working within a human services context, ideally within a multi-disciplinary team Applied knowledge and understanding of human services and care coordination Knowledge of the Child Protection Act 1999, or the willingness to learn Work well independently and can recognise and/or anticipate the barriers and challenges that young people might face Understand trauma informed principles of care and the importance of connecting the dots when working through assessment, planning and decision making Knowledge of social and community housing sectors, and the private rental market, or a willingness to learn and develop skills and expertise in these areas Demonstrated knowledge and experience working with young people and/or families exhibiting complex and multiple support needs Experience in supporting young people moving through adolescence into adulthood Experience in partnering with Aboriginal and Torres Strait Islander people and communities to offer support that is culturally responsive You'll thrive in a dynamic environment by being proactive and thoughtful in your response, rather than reactive to presenting situations You will look for the 'unusual suspect' using innovative and creative idea generation to achieve solutions to complex problems You will be able to build productive working relationships with a network of natural supports, professionals and stakeholders – we are all better when we work together.
Pre-Requisites National Police Certificate – or the ability to acquire Working with Children (Blue Card) – or the ability to acquire Child Safety and Personal History Screening Check (LCS) – or the ability to acquire - where the service and role provides care or support in a licensed care service Current driver's licence.
