Employment Type: Temporary Full Time (until 31 December 2025)Position Classification: Pharmacist - Grade 1 / Grade 2Remuneration: $ 80,513.62 - $119,173.77 per annum (+ super + 17.5% leave loading where applicable)Hours per Week: 38Requisition ID: REQ543833Applications Close: 30 January 2025Vaccination Category:AAbout Your RoleThe Transition of Care Pharmacist will facilitate continuity of care, optimise medication management and minimise the risk of medication misadventure for patients receiving transitions of care services, including patients receiving community care and hospital care at home.
The focus will be on supporting patients with respiratory conditions and heart failure during their transition home from hospital.This role will be responsible for optimising medicines management and patient outcomes and reducing medicine-related harm during and after transitions of care.
