About the Role Gympie's Children, Youth and Families are providing a service as part of its Next Step Plus and Extended Care Program and are seeking a Part-Time Transition Mentor - Out of Home Care to work 53.2 Hours per fortnight however hours may increase. The role will be based from our Gympie office. As a Transition Mentor, you will play a pivotal role in providing case management and support to young adults transitioning out of care and post-care. Your primary objective will be to help these individuals navigate the challenges of independence and adulthood by offering practical assistance, guidance, and emotional support. About the Service Part one: Next Step Plus program is for clients 15-17.5years and engages with the young person and their network to commence development of their transition from care plan Part two: Extended Care program is for clients 17.5-21 years with the focus on development of their transition plan and setting goals for the client, responding to immediate needs and providing support and guidance as needed, monitoring client budget Part three: Next Step Plus for clients 21-25years to provide check-ins, support and guidance as needed. Position Responsibilities Develop individualized transition plans focusing on areas such as accommodation, daily living, budgeting, education/employment/training, social connection, cultural connection, and family support. Collaborate with staff, government agencies, and other stakeholders to ensure comprehensive support for each young person. Offer support and facilitate smooth transitions into and out of services or programs. Assist clients in obtaining accommodation and accessing necessary support networks. Communicate effectively with internal and external stakeholders. Develop an understanding of cultural issues and ensure culturally appropriate support for Aboriginal and/or Torres Strait Islander clients. Participate in community engagement activities and maintain necessary administrative records. Foster a positive team environment by providing guidance and support to colleagues. Actively participate in staff meetings and contribute ideas for improving work processes. Undertake regular professional development to enhance your skills and knowledge. Required Credentials & Qualifications Degree in Health, Human Services, Behavioural Science, Social Work, Psychology or similar field Current Blue Card Current Licenced Care Service (LCS) Current Queensland Driver Licence Degree in Health, Human Services, Behavioural Science, Social Work, Psychology or similar field Ability and willingness to travel within the service region Ability and willingness to work outside of standard business hours and provide on-call support as required What We Offer: Competitive starting hourly rates ranging from $47.49 to $49.65, aligned with SCHDI Level 5.
