About the Role: We are seeking a compassionate and experienced person with a background in aged care to join our team as a Home Care Packages Case Manager. This role involves supporting elderly clients living at home, ensuring they receive tailored, high-quality care to maintain their independence and well-being. As a Case Manager, you will coordinate Home Care Package services, manage client assessments, and create individualized care plans to meet each client's unique needs. You will work closely with clients, families, and healthcare providers to ensure the best outcomes and enhance the quality of life for our clients.
Key Responsibilities: Assess client needs and develop personalized plans related to Home Care Packages. Coordinate services and liaise with providers, healthcare professionals, and families. Monitor and review care plans, making adjustments as needed. Ensure compliance with Aged Care Standards and relevant regulations. Provide emotional support and educate families on available services. Manage budgets and ensure financial accountability within care packages. Key Skills & Qualifications: Experience in aged care services or case management within Social Services. Knowledge of Home Care Packages and relevant funding models. Excellent communication, interpersonal, and organizational skills. Ability to work independently and as part of a team.
