Coordinate & Oversee Individualised Care Plans: Work closely with Care Coordinators and multidisciplinary teams to create and assess personalised, strengths-based, and outcomes-oriented care plans for each young person. Your leadership will help shape the support plans, ensuring they are responsive and evolve with the needs of the young people in our care. Facilitate Effective Communication & Collaboration: Establish robust communication channels across all sites to foster consistency and knowledge-sharing. You'll lead regular meetings with Care Coordinators to monitor progress, address site-specific needs, and create a collaborative environment that encourages resource sharing, problem-solving, and continuous improvement. Manage Resources & Service Integration: Direct resource allocation across sites to ensure a holistic care model, integrating educational, health, social, and recreational services. Your coordination will ensure that each site has the support needed to provide comprehensive, all-encompassing care for residents. Support Care Coordinators in addressing family concerns, keeping stakeholders informed of care developments, and fostering trusting relationships. Oversee After-Hours On-Call Support & Incident Management: Participate in a regional after-hours on-call rotation to support Care Coordinators during crises or after-hours needs. Lead responses to significant incidents, ensuring thorough investigations, and focusing on continuous improvement.
