We are seeking a Full time, Part time or Casual skilled Support Coordinator to join our team based in Belconnen, ACT. As a Support Coordinator at QuestCare, you will assist participants to work towards the identified goals in their NDIS plan by navigating the NDIS service system, making informed decisions, and obtaining the resources and support they need and want. You will need a good understanding of a range of disabilities and an ability to navigate the NDIS framework, seeking support and establishing relationships necessary to support your client to increase inclusivity within the community.
As a NDIS Support Coordinator, you will be responsible for interpreting participants' NDIS plan, assisting them in the planning and implementation of services as well as helping them gain skills to allow them to become independent in managing their own supports.
Using an individualized approach, you will plan, develop and coordinate services for NDIS participants by:
Establishing and maintaining strong relationships with customers and their families, carers and advocates, NDIA, other mainstream and disability service providers, members of the community, networking groups, funding bodies, government agencies, auditors and practitioners.Assisting NDIS participants in understanding their goals and the implementation of support services including management of funds and budgets.Planning, developing and building capacity in individuals to manage their own supports.Monitoring and reviewing plans and goals in consultation with participants on an ongoing basis.Providing support and assistance as appropriate to relevant parties.Organizing services and negotiating service agreements with providers.Arranging any necessary assessments that participants may require.Working closely with plan managers and notifying them of budget/breakdown of funding.Linking participants to mainstream or community services (i.e. housing, education, transport, health).Being the key contact for service issues, complaints, major changes and plan reviews.Working equitably with Participants with respect to each individual's right to dignity, privacy, independence and respect.Supporting clients to implement their NDIS plan to maximize the value for money they receive from their supports.About you: You have a minimum of 3 years' experience in a similar role ideally within a disability setting supported by the National Disability Insurance Scheme.Knowledge and understanding of the NDIS Quality and Safeguarding Commission framework along with the NDIS Code of Conduct.Ability to research autonomously and experience in information gathering and networking.Experience in working in a KPI billable hours setting.Demonstrated proficiency in the use of the MS Office suite, working knowledge of databases and demonstrated administration skills.Demonstrated experience in maintaining accurate, professionally appropriate and current records and written reports.A genuine, empathetic and passionate nature.Exceptional time management and organizational skills with the ability to prioritize competing demands.Ability to liaise and build rapport with participants, families and carers.You hold a current ACT drivers license, valid WWVP (with NDIS activities) and Covid clearance.Please note, only shortlisted candidates will be contacted.
